Insurance Appeal Form Letter
Occasionally, I'll hear about a patient having trouble with their health insurance company for approving a certain type of amyloidosis treatment. Usually, the application for a stem-cell transplant is rejected, although any of the chemotherapy options can be problematic -- especially if the hospital you go to doesn't know how to do through the hoops properly. Over at Boston Medical, they have a great insurance liason who knows exactly what to do when submitting a claim to an insurance company. Other treatment centers may not be so lucky.
I found this link on BankRate.com that provides you a outline of a denial appeal letter to send to the insurance company when the reject your request for treatment. This can work for any medical treatment request -- not just for amyloidosis. I hope this is helpful to someone.
